Leave of Absence Request During Term Time

We have adopted a zero-tolerance policy with regards to holidays during term time, this is inline with Local Authority and Government Guidelines.  Government legislation from 1st September 2013 states schools can only grant requests for leave of absence due to exceptional circumstances.

No holidays will be authorised unless there are very exceptional circumstances and will be reviewed on a case-by-case basis.  Parents should complete a Leave of Absence Request form and hand it into the school office.  This request will be discussed by the Priority Pupil team at their weekly meetings.  Whether the absence is authorised or unauthorised, written evidence will be provided to the parent by the Acting Headteacher.

Medication Consent Form

Only essential medicines will be administered during the school day at Holy Rosary. These will only be medicines prescribed by a doctor and where dose frequencies cannot be taken outside school hours e.g. four times a day.

Parents will be required to bring in the prescribed medication and sign our Medication Consent Form (available below or from the school office) before any medicine is administered.

Only prescribed medicines that are in-date, labelled, provided in the original container as dispensed by a pharmacist and include instructions for administration, dosage and storage are to be accepted by the school office.

Medication must not be sent to the class teacher.
